For those out there that choose not to use just a regular text editor (vi, emacs, editplus, etc) to code your PHP, but rather opt for one of the most popular WYSIWYG tools out there today, Dreamweaver MX, then we have a treat for you. Not only has there been a book published about using Dreamweaver MX with PHP, but CodeWalkers has a sample chapter from it.
This page gives you the overview of what the book is laid out like, with a brief introduction to PHP and MySQL at the beginning, followed by the "meat" of the book quickly after. This is not a book for someone just learning PHP, but rather a primer on how to use that PHP knowledge that you already have, and incorporate it with the power of Dreamweaver MX.
