Well, last week my little discussion question got overshadowed greatly by the release of PHP 4.3.0 (whee!). So, I'm going to ask it again to see if we can get some more responses.

What kinds of "horror stories" have you seen when you either first started at a new place of employment or when you started on a new project? Were their web servers a mess with no CVS kinds of systems? How much clean up work did you have to do to just get things back to being "usable" again?
